In January of 2016, Holy Saviour Parish began its Eldercare Program, which we are very excited about! From experience, we know that a good number of our parishioners and their loved ones are sometimes confronted with: situations involving aging; rehabilitation after sickness; assistance needed in the home for a loved one to remain in their homes and; possible nursing home placement. We know that these situations can be frightening and exhausting.

EldercareServicesHolySaviour450Our Parish was the first in Montgomery County to have this Program, offered in partnership with Catholic Housing and Community Services, and funded through a very generous endowment. As of July 2017, only ten parishes in the Archdiocese presently have this Program: two in South Philadelphia; three in Northeast Philadelphia; one in Bucks County, Delaware County, and Chester County, and; two in Montgomery County.

Mrs. Jennifer Rose, MSW, LSW, is in charge of this Eldercare Program for Holy Saviour Parish. She is a full time staff member, helping older parishioners to learn what benefits are available to them and helping them and their families to work with and understand the complex world of government agencies, healthcare and general aging services. Jennifer has a professional degree and a great deal of experience in this field, and she has been chosen by the Pastor to lead this Program from a number of applicants presented to him through Catholic Housing and Community Services.

The primary mission of this program is to help individuals stay in their homes safely and/or to support the transition to and from home, hospital, rehabilitation facility or skilled nursing facility. By providing information and in-home evaluations, a plan is created for the individual needs and preferences of the individual, couple or loved ones. As our Program progresses, we would like to be able to hire one or two parishioners to assist with this program, especially someone who speaks Italian.

Included in this Program are services such as:

* helping individuals decide the best Medicare Part D Plan.

* coordinating home health care services

* explaining the difference between home health care and home care

* working with the VAEldercareFamilyResources

* helping families complete applications for Medicare and Medicaid benefits

* coordinating medical equipment delivery

* working with social workers in hospitals and nursing/rehab facilities

* securing legal assistance

* arranging for in-home delivery by local pharmacies, and much more…

The Mission of Holy Saviour Parish Eldercare Program is to assist the older members of our Parish in enjoying independence and the highest possible quality of life in their homes and our Parish community. Jennifer has met with our Senior Club to explain the Program; met with our Extraordinary Ministers to ask them to identify needs that they see in the homes they visit, and prepared an informative brochure concerning our Program.
